However, the usual starting dose for methimazole is 30 mg daily. In some cases doctors start their patients on lower doses to see how they tolerate the meds and then they slowly increase the meds. After 6-8 weeks on meds, though, you need a lower dose since the thyroid hormone in your gland should have been used up. At this time most people are on a maintenance dose of 2.5-10.0 mg methimazole daily. You should be monitored with FT4 and FT3 levels since TSH can take a very long time to rise. TSH stays suppressed in active GD, so not to worry if it is still low; that would just mean that antibodies are present; it is the FT3 and FT4 that determine if you are in range, or " euthyroid, " and everyone has their own set point, which is the level at which they feel best. If you haven't had a TSI test, you may want to request it, as that is the only way to confirm the diagnosis of Graves'. It is also good to get Free T3 as well as Free T4, as these are the thyroid levels that most accurately reflect the available hormone in your system. Free T3 and TSI tests are expensive, so if you don't have insurance, you will need to decide how often they fit within your budget. TSI tests should not be repeated sooner than every 3 months, as it takes that long for the proteins that comprise the antibodies to degrade (so they will hang around for that long). I did quite well on Methimazole (the brand name Tapazole and the generic) with fairly active Graves Disease. I wonder why you were told to increase the methimazole if it seemed to be working. There are 2 possibilities: - that your dr. felt 10 mg. isn't potent enough and there should have been a more dramatic decrease in the FT4 - that your dr. wanted to start you off at a low dose to see if you can tolerate the meds and plans to gradually raise it. The second is a common MO but not a good one. Ideally the dr. should start you on what he thinks is a maximum dose, which with mild hyperthyroidism is likely to be between 10 and 20 and then gradually decrease it. If that was his plan, but he erred in starting you at 10, nothing to worry about and 15 makes it easier to take the meds in 3 even doses (the ideal, though I took mine twice a day not being aware of this). If you corner your dr. and he admits to the second being his game plan, I wouldn't be surprised if your dr. is one of those characters who goes by TSH primarily and planned to keep you at 20 till you showed signs of hypothyroidism. Please don't think I'm paranoid or neurotic but I've seen this happen too often. I have a friend who is on Tap, also with no or little symptoms but based on bloodwork alone. It's weird to be on something so potent when you're feeling normal but it's a good option, definitely the best the drs. will offer for hyperthyroidism. I start my bowel prep on Monday, and then off to UCLA (via Kaiser) on Wednesday. I have rearranged my entire life for this. I have put my condo on the market for sale and moved in with my fiance. This will allow me to not work for 6 - 8 weeks, and then to only work about 30 hrs/week afterwards. I plan to start walking as soon as I get home (just up and down the block) and have made room in my life for 2hrs a day at the gym, so I can reward myself with a nice steam room visit or jacuzzi afterwards. I've sent away for sample packets from Vitalady so I can see which ones I tolerate best after surgery. I've been using the Atkins Advantage powders in my pre-op weight loss, and like the vanilla & chocolate. I add frozen strawberries to vanilla and banana to the chocolate. Guest guest Posted February 22, 2006 Report Share Posted February 22, 2006 Janet, good for you for preparing in the way you have for this surgery...and how lucky you are to be able to reduce your workload so that you can focus on you, your health, your recovery, and your new healthy habits. In the beginning, you may find that adding banana to your drink winds up being difficult to tolerate. Everybody is different, but I know a lot of people who have a hard time with bananas initially. It's kind of odd, because I don't think of them as being overly sweet...but maybe it's the fiber and not the sugar? You should follow what your surgeon has to say about nutrition. At Richmond, they really advocate eating "real food" as opposed to protein shakes, because they think it is a more sustainable long-term lifestyle. The protein shakes can seem to be too much "diet mentality" and you can become rebellious or reactionary to it. There are others, very successful post-ops, who SWEAR by the shakes. It's an individual journey, I suppose. For me, I use shakes occasionally as meal replacements...and I occasionally rely upon protein bars, because they are more dense, and give you a sense of satisfaction. Drink your water, take your vitamins...be vigilent about not drinking while eating. Don't feel that you need to eat more than your tummy tells you to eat. Revel in the beginning at your little tiny tummy that can't handle much food. That will change with time, so take advantage of it while you can. Keep up the exercise. I got a bit derailed with my normal exercise routine because I became quite anemic for several months afterwards. I am back to doing Pilates 2 times a week (one on one training, so it's pretty intense), and I am trying to get more cardiovascular/weight training in again. It's important. As far as the changes in my life...they are huge. I went from your weight (261.5 when I started this thing, at 5'6"...wearing 18s and 20s) and now I'm 161.5, wearing 8s and 10s. I had my surgery on June 8th, and I haven't lost any weight since December (well, 2 lbs.). It's definitely slowing down. Technically, I need to lose 7.5 lbs. to not be "overweight" anymore, based upon BMI. But, frankly, I look pretty great at this weight. I'm toning up a bit more, and I would like to shed another 10 or 15, to give myself a little bit of wiggle room...and to get rid of the back fat and upper arm fat I still maintain. I'm lucky, in that my skin is just fine...and I'm nearly 42. At first, my stomach really bugged me, it was kind of droopy...and so did my inner thighs. But both are starting to look much better...and my breasts are fine. Ironically, although I lost a bit off of my breasts, they actually look bigger, because all of the surrounding fat is gone, so they stand out more. I have absolutely no posterior, so I'm working on regaining my muscle there. I used scar cream and vitamin E oil and Aloe Vera pretty much every day (after the first couple of weeks, when the doctor told me it was ok.) Consequently, my scars are already barely noticeable. I can easily fit on airplanes. I can borrow my skinny friend's clothes. I can walk into a Parisian boutique without being sneered at by the skinny French women who work there. I can buy clothes that are on sale...and I buy things that look really good...as opposed to things that look ok, and well...hey, they fit. When I walk into a room, and people are looking at me, I no longer worry that they are looking at me because I'm fat. (now I just worry about whether I have spinach in my teeth...hee hee.) I'm much smaller than my boyfriend. And even my ex, who is very very thin, has bigger thighs than I do now! I never get the "You'd be so pretty if only you'd just LOOSE weight..." speech anymore. When I look in the mirror, I never feel self-disgust. I can throw on a pair of old jeans and a tee-shirt, and look adorable, instead of like a slob. I was always athletic, and active...so my life wasn't all that impaired by my weight. However, it strikes me that as I dash up the four flights of stairs to my apartment, carrying my laptop and a bag of groceries...I am not even breathing hard. At all. When I went running the other day, the first time I've run at this weight...I expected to run about a block before I had to walk. Instead, I ran 3.5 miles around Lake Merritt...and my body was no problem. It was more my mind I had to battle (you know,"I can't run this far...I need to walk..."), but my body was just fine. NO more, 'if only I were thin..." running through my head. When I eat something that isn't 100 percent great for me...I don't freak. I just make sure that the next thing I eat is lighter fare, and healthy. When my scale goes up a lb. at my period, I don't freak. I just watch what I eat for the next two or three days...and it balances out and goes back down. Get this: I can feel and SEE a difference in my body when I am only one or two lbs. heavier. This feeling and sight spurs me on to action. I don't let it turn into 3 or 5 or 10 lbs. I used to marvel at people like that. I mean, to gain a lb. or two was NOTHING in my former life. I didn't hit the panic button until it was a LOT more than that. Now, I weigh once a day...and I balance my eating habits a bit. If my weight goes up a bit, and I'm sure I haven't eaten too much...I up my water, and I take a walk. I don't get depressed and say, "What's the use?" I feel more in control. I feel more in control. Thanks for the reply - I just want to clarify that I am using the drinks as PRE-OP weight loss only. I will not even be able to have full liquids for several weeks. As Sue in Palmdale was saying, UCLA is very strict about the pre-op and even more about post-op and we just don't move to anything like a protein shake for weeks. I'm sure that by the time I get to have something like a Lactaid protein drink it will seem like heaven! Thanks for sharing your stats, you sound amazingly happy and progressing very quickly. I remember dining at the little cafe right inside the Arc de Triomphe on Champs Elysee at my highest weight (~280) and as I got up to leave, the chair would not release my butt, so it went waving in the air with me. Everyone in the restaurant laughed and laughed along with them, but was dying inside. Can't imaging the snooty French women in boutiques not staring at me.....at least for today! Yikes, the chair/butt thing must have been embarrassing! Good for you for laughing, though. The protein shake to lose weight helps, I did that route, too. ANd as I said, many many post ops have had great success using shakes afterwards, but I haven't used them very often because the surgeons at Richmond, like the ones at UCLA are against it. In the end, you will need to find what works best for you...because we are all different. I am very happy, life is good...and honestly, I feel natural, now. I don't feel like an imposter in a thin suit. I feel like a thin person. It's a great feeling, and I know I will never let myself get "there" again...it was just too painful for me. This is a wonderful gift...use it wisely and it will serve you well. Deepest, richest blessings on you during this journey. It's a wonderful thing.
