Atypical Clubfoot Questions

Hey Everyone,

Do any of you have experience with atypical clubfoot treatment that

you could share? I read a post on another clubfoot board and I would

like to share which doctors you have had a positive experience with

for atypical clubfoot treatment. I think she said her child is on the

6th cast and a surgery may have been possibley mentioned but I'm not

very sure what kind or anything. I don't know much about " atypical "

so I don't know if they are treated a bit different then the regular

ponseti method treatment. Please share which doctors you have taken

your child too that have been successful with " atypical " clubfoot.

Plain and simple... go see Dr. P or even Dr. Dobbs. Best advice one can get

when dealing with atypical feet.

Hi ,

My daughter who is now 9 months old also was born with

atypical bilateral clubfeet. Atypical just means that the clubfeet do

not classify in the typical description of clubfoot because they are

harder to treat, usually very chubby and short and more stiff. About

5% of all clubfeet cases are atypical.

Dr. Morcuende explained to me that they call it " atypical " because

they just have not found a more technical word for it. My daughter

had a deep crease in the back of the feet (the heel) and the bottom

of her feet as well. Don't try to analize this with other doctors

besides Ponseti himself and Dr. Morcuende. They are the pioneers of

treatment for atypical clubfeet. Even the doctors that they have

trained are not experienced enough on atypical clubfeet.

I copied and pasted the following explanation from Dr. Ponseti from

an e-mail he sent to me in the past:

Yes, has what we call atypical clubfeet. Atypical clubfeet

occur in less than 5% of all clubfeet. These feet are unusually

stiff and very difficult to treat. Unless the knees are severely

flexed, the casts tend to slip. In addition, I am concerned whether

she has toe extensors. Does she bring her toes up when you tickle

the bottom of her feet? It is important to know whether the muscles

that move the toes up are functioning.

Doctors who have treated the usual type of clubfoot will often have

difficulty treating the atypical clubfoot since the treatment plan

must be altered slightly. Many physicians are unable to obtain full

correction of the deformity and recommend that the feet be treated

with major surgery. Only a heelcord tenotomy under local anesthesia

should be performed. If other surgery is done, this type of clubfoot

forms scars which make the foot more difficult to treat.

Once the feet are fully corrected and ready to go into the brace, the

Markell shoes which most physicians use are not satisfactory for the

atypical clubfeet. The Markell shoes do not hold the feet securely

and often the babies have problems with sores from the shoes.

We have treated several dozen of these atypical clubfeet successfully

and we have a local craftsman, , who makes a sandal type

shoe which works very well. If Dr. Grogan has not treated the

atypical type of clubfoot successfully, I strongly recommend that you

bring here. There is a Mc House near the

hospital where most of our out of state families stay. I would then

plan to change her casts every five days on a Monday-Friday-Wednesday-

Monday-Friday-Wednesday schedule. Until I have casted her a few

times and can see how she is responding to treatment, I cannot

accurately estimate how many casts she will need.

We trusted Dr. Grogan from the Shriners Hospital but after 5 casts

her feet did not look corrected. I took my daughter to Ponseti's and

Dr. Morcuende and after 4 more casts they were beautiful. Now I am

taking her again because she will need another tenotomy since the

achiles tendon got tight again. Hopefully that will put us at the end

of casts and we will not have relapses (comply with the brace for

sure).

I second that... and anticipate purchasing shoe with Ponseti brace. WE

have to budget carefully for the 300 $ every time Gabe needs a new pair.

My daughter has unilateral A-Typical Clubfoot. The only one I would trust is Dr.

Ponseti himself. She had seen 3 Dr.s that all said her foot was corrected when I

contacted Dr. Ponseti. We had to fly to Iowa to get her foot fully corrected

because she developed complex deformities (Plantaris/Cavus) because the method

does need tobe slightly altered and it needs to be done correctly! She is in her

last cast now that comes off on Halloween. I will be posting her Iowa lovely

expierence when we get settled in our new house...

Dr Ponseti gave me a list of doctors qualified to treat atypical clubfoot, but

it is currently burried on an old hard drive, so I am winging this a little bit,

but here goes:

Anyone at U of Iowa

Dr Colburn, Pleasanton, CA

Dr Herzenberg, Baltimore, MD

Dr Dobbs, St Louis, MO

Dr Mosca, Seattle, WA (really, dont hold Kai against him! lol)

Outside the US -

I do not remember this list very well, so it probably is not comprehensive and

is missing some docs:

Dr Pirani, BC, Canada

Dr Marc Sinclair, Hamburg, Germany

Dr Ey, Barcelona, Spain

Dr , Manchester, UK

Treatment does differ a bit, so it is very important to see a doctor that is

qualified to treat atypical.
