Guest guest Posted August 28, 2007 Report Share Posted August 28, 2007 Dear migrant health research colleagues, The Sentinel Centers Network (SCN) is a representative sample of Community Health Centers located throughout the United States. Health Centers often are the primary source of health care for some of the nations most vulnerable populations. The SCN 2005 Annual Report provides information on nearly 1.6 million health center patients. This report provides information about patient demographics, most commonly diagnosed illnesses, chronic health conditions, service utilization, and procedures and screening rates among the patient population. Information related to disparities in health and services are also presented (e.g., by age, gender, race/ethnicity, poverty status, and payment source). The Sentinel Centers Network is funded by the Bureau of Primary Health Care at the Health Resources and Services Administration and conducted in partnership with the Michigan Public Health Institute.
