(without saying what surgery)? >> I'm also working for a small company...I didn't need a letter for my employer...and I didn't need a letter to go back to work. I just said I'd be taking my two week vacation on xx days and that the first week back would probably be part time due to surgery. However I did tell everyone what I was doing. I don't see any reason why your company will find out about the surgery because unless they are self insured...all they get are the premium bills each month.
