I agree with the rest of the group. I have had mine placard for

four years and just had to get it renewed. I had to pay nothing and only

had to get my doctors to fill out the form. My primary care doctor filled it

out for me. It is very handy. I suggest you fill out one as soon as

possible. I know how you feel when you are too tired to hardly even walk

in the store more less walk several feet to the store.
