I think the 18 month window mainly involves the reduction of the malapsorption factor --- ONe's intestines adjust somewhat to the surgery by that time. The common channel may lengthen, the cell wall grow thicker, etc. to maximize absorption and therefore we 'level out' at a certain weight. It is true that our stomachs may 'stretch' to some degree but I don't think the stomach ever returns to 'normal' size --- 2/3 of it was removed and that is very permanant (thank GOD for that).
