You get the form at your local health department, fill it out, they sign it

and you turn a copy over to the school or anyone else who needs it. Of

course they will try to talk you out of it and condemn you before they give

you the form so just stick to your guns.

I can e-mail you the exemption form that I received. They accept copies, two

friends who home school received theirs from another friend who e-mailed it

to them and then they submitted it to their doctor and school. No problems.
