Reflux meds!!

Hi

can anyone tell me why Dr H uses losec in the am and zantac in pm,

to treat reflux, at least this is what she suggested for Liam to try.

Unfortunately our gastroenterologist hasnt taken to well to Dr H

suggesting any treatment for Liam, and, thinks these 2 drugs

shouldnt be taken together. I vaguely remember Dr H having a reason

for doing this??? any ideas???

For the moment we have started Liam on Zantac twice a day. The other

thing about Losec is that it is in tablet form(in Australia) and if

you crush it (apparently) the acid in the stomach breaks it dowm and

it doesnt work properly. So if there is kids with g tubes, who wont

swallow a tablet ?? how do you use this drug.

After coming home from America, so full of hope and ideas that might

get Liam eating, we have come down to earth with a thud, at the

attitude of Liams gastro specialist to all we learnt, why is life so

hard! id like to think this Dr is working only in Liams best

interest, I cant help feeling there is proffesional rivalry, holding

us back. This Dr thinks endocrinologists should not be treating

gastroenterology problems.

Hi Jodi,

Sometimes just one medicine twice a day isn't enough to stop the

reflux since they work differently, Losec is a PPI and Zantac is an

H2A, someone else can explain better than I can how they work

differently! (Pat or Jodie Z). Olivia was on Losec(prilosec) in the

AM and Zantac at night but we're now trying just the Losec twice a

day. They can't be taken together at the same time but they can be

taken at different times of the day. We did an equal split with the

doses at 8AM and 8PM. Our pharmacy also compounds the Losec for us,

turning it into a liquid form. Phone around to different pharmacies

if you can to find one who will do this.

I know just how you're feeling with the Dr's... when we got home

from the convention our Dr wouldn't even look at the paper from our

Appt. with Dr.H. I get the impression that he's memorized the one

page in the book about RSS and has lumped all of the kids into it.

He told us that " all RSS kids have reflux, delayed gastric emptying

and GI problems and there's nothing you can do " ... this was in

response to me wanting the PH probe and the GET tests done. He also

said that pyloro's and fundo's don't work so there's really nothing

you can do. Granted, I know that Olivia is not severe enough to

warrent either of those surgeries but I was very dissapointed that

he has generalized all RSS kids like that when we know that every

child is different. He also thinks that Olivia's weight is just

fine even though she no where near to being on the height to weight

chart, she needs to gain 8 lbs to make it to the 3%ile line and

that's if she doesn't grow any taller! We may be Dr shopping yet

again depending on how it goes with the next weigh in in Sept. We

do have to keep this Dr. long term though because he's the only one

qualified to do her Pamidronate IV treatments for the OI.

Best of luck to you with your Dr. If you find something that

works to change his mind please let me know! It might work for our

Dr as well and Dr shopping is not my favorite passtime!

Jody - Here in the US we also see that most pediatric

endocrinologists treat height only and GI doctors treat weight

only. For RSS/SGA kids, this fails to see the intrinsic link

between the two! Dr. Harbison is the first to admit she isn't

trained in GI, and refers children often to GI doctors. However,

because she has seen 300+ RSS/SGA kids over the last few years, and

has about 130 in her current patient load, her experience with the

most basic of the GI issues gives her a lot more experience than

many GI doctors!

I don't know what Losec is -- is it a proton pump inhibitor?

If it helps, our GI doctor AGREES with Dr. Harbison -- you have to

give the proton pump inhibitor at least 12 hours AFTER the Zantac

though. So that is why she (and our GI doctor) prescribe the proton

pump inhibitor (for us it is Prevacid) in the morning and Zantac

with dinner.

Also, I don't know about the tablet. Prevacid comes in a capsule.

I take a spoon of chocolate ice cream, open up the capsule and

sprinkle the tiny medicine bits on top of the ice cream, and hand

her the spoon, of which she promptly swallows. Easy as pie. Could

Losec come in capsule? Is there another proton pump inhibitor in

Australia?

We used just Zantac (twice a day) for 2 years. It wasn't until we

switched to the proton pump inhibitor in the morning and Zantac at

dinner that 's reflux truly got under control.

I am so sorry that your GI doctor is getting defensive. Is there

any way that he or she would be willing to correspond with Dr. H via

email? I have been VERY impressed with how delicately Dr. H handles

her dealings with other doctors.

losec is prilosec and is a proton pump inhibitor.

Did tolerate her meals or improve her appetite when she had

her reflux under control?

Our Dr has agreed to a trial of zantac and only if he shows an

increase in appetite or tolerates more formula, will he order any

tests eg an endoscope.

I have given this Dr, Dr Hs email, I dont think he will contact her,

he is adamant Dr H could not know Liam on a half hour visit, totally

frustrating and he just doesnt seem to understand the extent of her

work with these kids. I guess we just go slow, if we can make a

difference with the zantac, maybe I can convince him to go another

step.
