Guest guest Posted August 10, 2004 Report Share Posted August 10, 2004 Hi again , You would just have to bring her sugars back up by feeding her, carb/protien combos are the best thing, refined sugars are not very good for this (including juices), Pedialite helps too. Since their bodies are so small they sometimes can't store enough sugars to sustain a long fasting period so they run out. If 's sick and spilling ketones in the high range and cannot eat due to high fever, vomiting, refusal to eat when sick, then your best choice might be to take her to the hospital for a D-5 or D-10 IV drip. This is a 5% or 10% dextrose drip that will help replace sugars until she is eating again. Our Dr told us that the difference between feeding and an IV for bringing sugars up is only 1/2 an hour so we can usually convince Olivia to eat and drink a little something since she hates IV's. She's not terrified just scared of the poke since she gets them every 3 months for treatment for her bones (non RSS related). As for her diet, I'm not too sure there, every child is different. We try to cut out as much refined sugar as we can and let her eat/snack very often, about every hour or whenever she want's to eat. Remember that I'm not a Dr and this is just what works for us, go ahead and check with your Ped. to see what they like to do in these situations. It's important that your daughter's diet consist of as many complex carbohydrates and proteins as possible (as opposed to simple carbs.). I don't eat any white bread or pasta (I love the South Beach diet craze b/c it means that whole wheat pasta is a lot easier to find than it used to be ). If I do break down and eat a serving of something that's considered a simple carb (anything with white flour and/or sugar) or a starch, I always make sure that I have protein with it. It's also important that your daughter eat often. Pedialyte does not have enough sugar in it to get rid of the ketones, but Gatorade does. Drinking is very important as our kids will get sicker from dehydration that goes along with the spilling of ketones. If that does not work, or she does not take any liquids, then the standard course of treatment is to have her go to MMH's ER and be rehydrated with D10 - NOT D5 as the residents may try to do. We have had lots of experience with this over there and I know what they try to do. The nurses really understand better because they have seen Max so many times, but even then you can get a nurse who has not met Max and does not know the procedure. If is vomiting, then you should call Dr. Rosh and let him take the action that is necessary. I don't know if Dr. Cohen, et al, knows the routine or if he would give you a hard time with it. Dr. Rosh and Dr. Yuseff are really familiar with the protocol. The other thing you can do is CALL ME!!!!! I don't mind a call even if it is in the middle of the night. I can advise you and guide you in the right direction. You should also request from MAGIC the piece that I believe Dr. H. put in their library with the guidelines for this situation. By the way, I spoke to Yadira this morning to schedule Max's Sept. appointment and she told me that you were on her list of people to call when Dr. H. is taking new patients.
