New Form for anyone seeing Dr. Harbison

Hello everyone -

Dr. Harbison has finally signed off on a form I created to help

families and her at visits at Mt. Sinai. For anyone who has a first-

time or returning visit with Dr. Harbison, please email me privately

and I will email you the form. It is standardized, and has you list

medications, any illnesses, etc. for the prior months, your top 3

questions, and then any other questions. She will fill out the

height, weight and head circumference.

She is asking that this be filled out and sent to her ahead of your

appt. I will confirm if she wants it by fax or email.

Then bring TWO copies of it - one for her " just in case " and one for

yourself. You can write the answers to your questions, and record

the measurements, etc.

Hopefully this will help her stay on track, and help parents too.
