Guest guest Posted November 25, 2000 Report Share Posted November 25, 2000 Hello, I hope there is someone out there that will answer my question, I have a what I hope isn't a silly question, when you went to your primary care physician for the history and physical exam did anyone take a copy of the DETAILED guidelines for Taking a History and the form Dr R provides or did you just tell your dr that a complete history and exam is required? I don't want to insult my dr by telling him his job. I go Monday morning to him. Also, is everyone taking the Citrucel or is anyone taking the chewable Fiber Choice? The thought of Citrucel everyday for the rest of my life is not a pleasant one! One I call the Dr. R notebook that has a copy of his paper, the patient manual, and instructions on all aspects of the step process to get the MGB. I put all information under tabs so when I needed it to show a Dr or talk with insurance Co I had all the info together. The other notebook I call the changing me notebook. It has tabs and all of the completion(documentation) of the steps for MGB. I got copies of all medical records from Dr I had surgery or lasting visitation...ie the ob-GYN , cancer reports, etc.. What I did is take a copy of the history and physical exam and filled in the basics, like address, name , weight and she filled in the rest. Dont feel like your insulting them, my doctor thought it was a great idea and actually commented about the good Dr. Rutledge having everything in order for patients. I explained how I wanted him to participate in the process, and how I would need his help after surgery if there were in minor complications, like infections, or stables removed etc. He seemed happy that I had that copy. Then he knew exactly what was needed of him, and was able to tell me at that time whether or not he could assist me as needed. I actually retyped all of this information (made it a chart) and my doctor filled in the answers. I could send you a copy of the chart if you like.
