Guest guest
After we send in our patient information form, shouldn't we hear something from Dr. Rutledge? I sent mine in at least 3 weeks ago and the only thing I received was a confirmation. Wondering if I should go any further with this process until I hear something. If anyone has an answer please let me know. Thanks. Pat

I think I sent in my patient form just as he was switching to this procedure as I got a letter from him stating: Thank you for submitting your patient information. As a new part of our process we are not sending your insurance letter until we have competed our review of your application packet. http://clos.net/preparation.htm This letter was dated 11/5/00 _______________________________________________

I've got my patient letter done, I need to set up a couple of apts. Thanks again.
