H&P only or copies of your medical records as well??

Send all significant medical records too! Not colds and sore throats but

anything that may have a bearing. I sent my endoscopy and colonsocopy

reports..all of them...as well as visits which showed my high blood pressure,

orthopaedic consultation, etc.
