If not ask them to fax it to you. I did. I have United Healthcare. They did not even require the letter from Dr.Rutledge, just the referral, height,weight and BMI. If they don't ask for the letter I wouldn't bother sending it. Make sure you have something in writing from them. I think Dr. R is having putor problems and that is why he sent quadruplicates!
