I have Aetna but I am in California. What I did is once I got Dr. R's

insurance letter I called the pre-certification number on my insurance card

and gave them all of the information (codes, etc.). Once I did that they

gave me an address to overnight my letter to. I also got the name of the

Supervising Pre-Cert RN so I knew who to call to verify it was received.

I had absolutely no problems with Aetna and was approved in 1 week. Good
