I am glad that she will be seen by someone tomorrow. This reminded me to

encourage you to have her keep track of her physician visits. This is important

for disability and disability reviews. I learned this during my last review.

All they need is:

Date / MD Name / Reason For Visit

They do not need the address or any other contact information for the

doctors. Also any contact counts as a " visit, " i.e.; phone calls, pick up

scripts

if you speak to a nurse, doctor, or pharmacist, etc.

Re; Reason For Visit should be kept simple, i.e.;

Pancreatitis, Pancreatitis- Blood work or Pancreatitis - CT Scan,

Pain Management (whether a visit, request scripts, pick up scripts),

Depression (for any type of therapist, it doesn't have to be a doctor or

psychiatrist).

It is important to remember what your disability was based on. My disability

was based on Pain and Depression, not pancreatitis. Therefore it is more

important for me to focus my doctor's visits on these tow things and not say

pancreatitis.

On the other hand, it would behoove you to include any and all medical,

dental, and psychosocial appointments or hospitalizations that occur. My list

was

so comprehensive, they canceled the review based on what I submitted. I did

not lie of course, but included literally everything. I actually contacted my

hospital, lab, doctor's, and therapist's office to coordinate dates and

times, so I made sure they were all correct, and ... so I did not leave one

out.

I thought I would share this since it is easier to keep it up as opposed to

going back and creating it from the date of disability or before.
