I didn't even know that there was a form...but I was asked questions along that line when I first applied. Incidentally, I was turned down the first time applying for SSDI...now I have a lawyer and going thru the hearing process. The lady that took my application was really nice and compassionate...she couldn't believe the amount of information, doctors, numbers... I had everything for her forms, I was " most together " she had ever seen anyone when applying. My lawyer said that unfortunately, they only saw I was only 34 and college educated...and I still work 10 hours a week (at least I try) and I had a boss who paid me my salary (very generously) for nearly a year even though I didn't earn nearly a quarter of it. My diagnosis with 3 doctors all said the same thing...chronic pancreatitis and abdominal pain. I hope that someday that this diagnosis (CP) is automatically approved...no delays, no turn downs... cuz for all the awful things we live with, wouldn't that be nice to not have to fight the system ...? Thanks again for sharing your info. I don't want to discourage people in applying for SSDI....I just know that in my case, I didn't get approval and I had support from three doctors working to help me. I am out of work and on leave from my school system, therefore I have been on Short-term Diability and now I am on Long-term. I teach special education and I know parents apply to claim SSDI for their children's defecits and disabilities. I am (again) confused about the who, what, when, hows of SSDI. Thanks for the help! The SS definition of disability is very specific and important. Your doctors' evaluations must conform to this to do you any good. The applicant is responsible for providing medical reports. Do not assume your doctors mail out anything you give them; follow up. I think Heidi, for one, has had experience with this problem. What has me really up-in-arms is the Listing of Impairments - these are the codes that Social Security uses to determine eligibility. Look up 5.00 - Digestive System - and read the information there. Notice that chronic pancreatitis is only mentioned in connection with malnutrition or weight loss, with referral to code 5.08. According to the charts there I would have to weigh 91-97 pounds to be disabled! There is other information, too, so read through carefully and don't just go by what I'm saying. But these criteria are very, very difficult to meet, I think, and may be why so many cases are turned down. I'm willing to mail out copies of that medical assessment form to anyone who wants to privately e-mail me their mailing address.
