Guest guest Posted September 12, 2005 Report Share Posted September 12, 2005 Dear and anyone else interested in the CP Medical Assessment Form: I posted about 3-4 weeks ago when my co-worker who handles SSI and SSDI appeals showed me a form from the manual that attorneys use for fighting Social Security. This is NOT an offical Social Security form. It also has several problems that I think PAI should address. This is why I mentioned it and waited for response. For example, there is a list of symptoms which includes " poor appetite with weight loss " which isn't always the case, and if Social Security insists that CP must cause weight loss, as is reflected elsewhere in the form, what about many who both lose and gain weight, or who have gained a lot of weight, or who never lose weight? Another symptom listed is diarrhea; but not constitpation, which can be more of a problem, don't you agree? The form is for the doctor to fill out and send to Social Security along with all treatment notes, lab tests, etc. If you and your doctor fill it out together, you might get a positive result. If you leave it to the doctor (or his receptionist, as often happens) it may not help you as much as you'd like. A foreseeable problem in having all your doctors fill out this form independently is that one is liable to cancel out another. When have doctors ever agreed on everything in your case? I have mailed out copies of this form to anyone who has requested it and sent me their mailing address. I'd be happy to do this. Please let me know if there are any further question. Guest guest Posted September 13, 2005 Report Share Posted September 13, 2005 Please put something in there about the enzyme levels not changing Please, I have been accused of not having pancreatitis because there is no change in the level, duh this started in 1996 they don't start at the beginning of the chart and read, and have the time it is found at another hospital that they don't have the records from anyway. That is my two cents.
