Help !!

I go for my physical next week and know I saw a post earlier today where

someone wrote a letter about their history/etc. to help the doctor write his

letter. My doc is great but I know they don't always have time to sit down

and write this type of letter on their own. Could someone share with me the

checklist they made for their doctor and/or the sample letter. I have all

the stuff from Dr. Rutledge to take but want to make this as easy as possible

for my PCP.
