There are about seven approved here in the US. There are two types of WLS's. One is restrictive, and the other is restrictive with a malabsorbtive component. The one with the restrictive element only usually has a 1-2 oz pouch. The more malabsorbtive surgeries usually have larger pouches or stomach remnants. Those who have massive amount of weight to lose usually need both elements. So, a distal bypass is really what we have in the MGB and our pouch is about 4-6 oz to more closely allow normal eating patterns. In heavier patients, he bypasses up to 8 feet. The BPD/DS is also distal, but much more so. The advantage of distal is more malabsorbtion of calories and fat, but is also displaces nutrients. So, we take supplements for life to assure nutrition. Depending on your weight, health, age, comorbs, etc., you should choose the surgery that is best for you. It may be the MGB or it may not.
