Call me if you need help with BC/BS of AL. From memory, you will need to send the following to gain approval: 1. Letter of medical necessity from Dr R. 2. A letter of medical necessity from your PCP (I will show you mine) 3. A letter from you detailing how obesity affects your quality of living (see mine) Make it real sappy...draw tears if you can. 4. A DETAILED diet history going back at least five years. (Embellish if necessary) They need PROOF of one physician supervised in the past two years. (Meridia, phen-fen, Zenical, Opti-fast, etc) 5. Medical records from your PCP showing you have been twice ideal weight for at least two years. 6. Results from nutritional consult. (new requirement) Send all of this together or they will deny you until they get it all. And they will not advise you of what they need. I have the name and address of the physician who makes the final decision. But, I am not at home now. Dinah Boatwright has the name to.
