Ok. I have medical release forms from my gyn/onc, the hospital where both of my

surgeries were done, my gastro doc. I'll be asking you all questions as I fill

them out probably:)

Instead of sending a release form, the rad/onc sent my records, but it's the

same thing my gyn/onc is going to have and I was happy to see the operative

notes from my hysterectomy included. However, I want copies of ALL my records:

the field of radiation, strength of rads, what was shielded, etc.... How do I

ask for those types of records from the rad/onc and sound professional? help.

Just call the doctors' office, or whatever office you are wanting the records

from, i.e. hospital medical records department, etc....and have them mail you a

medical release form to sign. You fill it out and mail it back to them and they

will mail your records to you.

It's my understanding that you have to be pretty specific as to the fact that

you want EVERYTHING .... especially the doctors' operative notes where he makes

notation of everything he does and sees. I kinda laughed at the end of that one

where they counted all retractors, sponges, instruments and counts matched

preop. Course, I'm glad it did!!LOLOLOL

We live 100 miles from my health care so that is why I am doing it by phone and

mail. If you live close by, just walk in and tell them you want copies. I

believe the law states that you are entitled to complete copies upon request ...

no charge.

I can attest to the statement made about being VERY SPECIFIC about the

medical records that you want copies of. There is no more difficult place to

get one's records from than a military hospital... My husband had no

trouble but this is what I went through.

Right after I was released from hospital back in Oct 04, we went to the

Patient Administration and filled out the form. Needless to say I did not

state INPATIENT RECORDS so I did not NOT get anything involving my surgery.

On the average it too about 3 months to get each request submitted... Okay

now for Round 2... We requested IN PATIENT record for this date to this date

to include the log of meds given during the hospitalization... Got

everything but the log of meds. So then I said heck with it.. Okay now here

is it nearly a year later and what do I get in the mail but the In Patient

records TO INCLUDE the log of meds give while in hospital.. Wonderful. I

cannot believe that someone finally read the instructions and actually did

what was requested of them. Talk about shocked....

i went to cancer clinic where i had the raditation and chemo done had sign

a form got report in in mail in one week.
