seeking information on promotora models

I am seeking information about successful promotores de salud (lay health worker) models/programs. In particular, examples of job descriptions, and whether promotores are paid employees or volunteers (and rationalization for either one). If they are volunteers, what non-monetary incentives have been provided to encourage people to serve as promotores? Any information you can send would be greatly appreciated, including evaluations of what types of promotores models are most effective.
