Have you seen Where there is no Dentist by the Hesperian Foundation? It is very good. Attached is a very basic outline I made up, using references to that book. I have forwarded your email to Magda de la Torre at UTSan because she has developed a curriculum like what you are looking for, but I have never seen it... Let me know if you find something!
