Guest guest
Aetna never asked for anything at all. When you fill out Dr R's online form, it goes into a database that automatically takes the info and plugs it into a form letter. The letter is about 14 pages, and it uses every answer you gave and makes it sound like you're dying! Anyway, that was all Aetna ever got about me! I do have documented medically supervised weight loss attempts, but only 2--and 1 of those was only for 3 months! My BMI was over 40 though, and I had high blood pressure, high cholesterol, sleep apnea, and arthritis.
