Question RE: when to fill out form online

Okay, I was thinking I would be premature to fill out the online form without

first visiting my PCP regarding this surgery. Or should I go ahead and fill

out the form and get the insurance going before I see my PCP? How have you

all done this?

My PCP is a new doctor whom I have never seen... we recently changed

insurance and had to choose a doctor when we signed... I haven't been in to

see him yet. This is why I was thinking I'd need to see him before he

receives any info from the insurance company or Dr. Rutledge's office on

someone he has no knowledge of.
