My PCP was draging her feet (no time) writing my letter so I wrote it for her and took it to her office and ask her to sign it.I also sent medical records from 5 years back with my packet. In her short letter I stated how long I had been her patient, diets, basic health problems and stated she had agreed to support me and be my physician after the surgery and would keep close monitoring on vitamin & mineral defiency. I also only sent one family support letter (my husband). Hope this helps & good luck. Marie
Guest guest
Hi Joni, My doctor just wrote I was a morbidly obese female who had tried various ways to lose weight, then he listed some of them and gave my success rate (or lack of )) He then wrote that he felt this was the best alternative for me. And he would be very interested in following me after surgery to see the results. It was about 2 paragraphs long and he wrote it out by hand! Dr, R took it so it must have been acceptable. Hopes this helps, Rosemary MGB 7/24/00 Dr's Letter of Support > Hello Everyone... > > My name is Joni Shelton and I'm in the process of getting my packet > together to send into Dr. Rutledge to try to get approved for the > surgery.
