Hi Jonie,

My name is Marie and I had the MGB 6/2/00. My PCP was draging

her feet (no time) writing my letter so I wrote it for her and took

it to her office and ask her to sign it.I also sent medical records

from 5 years back with my packet. In her short letter I stated how

long I had been her patient, diets, basic health problems and stated

she had agreed to support me and be my physician after the surgery

and would keep close monitoring on vitamin & mineral defiency. I

also only sent one family support letter (my husband). Hope this

helps & good luck. Marie

Hi Joni,

My doctor just wrote I was a morbidly obese female who had tried various

ways to lose weight, then he listed some of them and gave my success rate

(or lack of :o)) He then wrote that he felt this was the best alternative

for me. And he would be very interested in following me after surgery to see

the results. It was about 2 paragraphs long and he wrote it out by hand!

Dr, R took it so it must have been acceptable.

Hopes this helps,

Rosemary
