RE: Randah Whitley - exercise as CPT

Hi , we met with Randah at Chapel Hill last year after we moved to

North Carolina. She does indeed advocate strenuous exercise as a

replacement for traditional CPT. We have done this sometimes by

substituting exercise for one of the CPT sessions of the day. Randah

likes trampolines or jumping on a bed, running in the yard (tag, etc.),

or pretty much anything that has Kate breathing hard. This sounds good,

but the hard breathing has to be maintained for about 15 minutes or so.

And, most kids will want to stop sooner - that was our experience

anyway. So (as Randah warned us) it was a workout for mom and dad too!

We really had to push Kate to keep her moving and breathing hard. It's

nice though on days when Kate has just had it with CPT.

I tried to find Randah's email address, but the UNC websites seem to be

kinda bonkers right now. They have a notice posted about having lots of

computers down due to the recent viruses and worms. Anyway, you may

want to call the UNC CF Center at .

Does anyone have any information on exercise as a form of airway

clearance? I was told this was a presentation given at the CFRI

conference but they will not have copies of it for a few months. They

said it was given by Randah Whitley a PT from NC. It sounds like they

use this instead of or along with the traditional cpt. If anyone has

any info on this method, please let me know.

Thanks for the info and the #! I am going to give them a call and see

if they can send me more information. My daughter is 9mo and I was told

she had exercises for babies as well. It just seems like it would be

nice to do in addition to the CPT treatments. She is crawling now and

it is so hard some days to make her sit still. I don't feel like I do a

very good job at times. Do you feel it is as effective as her CPT

sessions? Regardless, I guess it can't hurt.

Regarding the effectiveness of exercise vs. CPT sessions. I believe

exercise is generally more effective, judging from the coughing. Kate

doesn't usually cough during regular CPT, but with hard breathing

brought on by running and jumping she will almost always cough. I

should add that the exercise must be sustained for at least 8 - 10

minutes to produce any coughing. Let us know if you get more detailed

info from Randah.

I agree. This has been our experience too, he coughs more from running

and jumping than he does from any kind of airway clearance. At this

point, we only do airway clearance (via PEP mask or CPT) when

isn't well enough to run around.

It was really neat to see the list of exercises to do with them; thank

you for posting that. does a lot of this stuff (jumping, hanging

from our fingers in the air) on his own initiative. I continue to marvel

at how in tune he is with his own body's needs! But now I know to

specifically encourage these behaviors.

What is a PEP mask and how is it used for airway clearance?

It's a mask with a valve attached that provides resistance when you

breathe out. PEP stands for positive expiratory pressure - as you exhale

you exhale against pressure or resistance.

This lengthens the exhalation and keeps the lungs open, which helps move

secretions. You do a certain number of breathing cycles with the mask

on, then remove it and " huff. "

It has proven to be at least as effective as CPT. My son loves the PEP

mask and hates CPT, and I have arthritis in my hands that make cupping

them and keeping them cupped very difficult.

does not nebulize any medications but if you did, they make PEP

set-ups that can be attached to the nebulizer so you can do nebs at the

same time.

I would love to know this info you find as well. My little one is

not crawling yet so not too active. He is almost 8 months old ( Colgan)

We do CPT but he really fights me and I would love to do some of this

with him in addition.
