Guest guest
Ranee, I didn't take the history and physical form to my Doc. He examined me, did the blood test, EKG, and based on my history with him he wrote Dr. R a letter medically clearing me for surgery. He just wants to hear from your regular doc that you are healthy enough to undergo this major surgery. You have already provided lots of the info when you send your first form by the internet. He also wants a doc to commit to watch you and monitor you for vitamin and mineral deficiencies.
