open for the 19th Annual Midwest Stream Farmworker Health Forum

Each paid registrant will be

provided with a conference program and bag, admittance into all workshops, discussion

groups, a Welcome Reception, a Dinner Reception, a daily breakfast, snacks, and

beverages.

discount. Every 3rd registrant qualifies for a FREE registration with 2

paid registrations. ALL registrations must be from the same organization

and ALL registrations must be submitted at one time to qualify. Visit our

Cancellation: If you

cannot attend, you may send a substitute. Full refunds will only be

provided if cancellation is prior to November 4th. No refunds provided

after November 4th.

20th, but will go up to $325 after that date!

Rates & Reservations

Call the hotel directly at 800-292-7704 or 956-761-6511 and

identify yourself as a group member of " National Center for Farmworker

Health Forum " when placing reservations. Room rates for Cabanas

which provide Single/Double occupancy are $95/night plus tax and Condos (up to

4 occupants) $180/night plus tax. These rates will be available until October

20th. Rooms are limited, so make your reservations early.

Services

South Padre Island is located

off the southern coast of Texas and has two main airports for air travel. Each

will require the need for a shuttle bus, taxi cab or car rental to travel to

the island.

Information

The Valley International Airport

(HRL) located in Harlingen, Texas services South Padre Island with daily flights

from Southwest Airlines and Continental Express. Approximately 45 miles to

Brownsville/South Padre Island

International Airport (BRO) located in Brownsville, Texas has daily flights

from Continental Airlines. Approximately 25 miles to South Padre Island. Visit www.flybrownsville.com for more information.

Harlingen Airport Shuttle-There will be a

Harlingen/South Padre Island shuttle from/to the Isla Grand Beach Resort and

the Valley International Airport in Harlingen, Texas. The rate is $20

each way, or $40 round trip. To reserve your spot please call

Brownsville Airport Shuttle-Rio Gulf Coast

Shuttle Express operates out of the Brownsville Airport with a shuttle leaving

every 45 minutes from 6:15 am-6:35 pm each day for $2. Take the shuttle outside

by the Taxi Stand to Las Palmas Plaza in Port Isabel, then connect to the Wave,

a free shuttle service that runs along the beach, stopping at hotels and

attractions. The Wave operates from 7:00am to 7:00pm every day of the week, and

leaves from Las Palmas Plaza at :21 and :51 minutes past the hour.

Taxi –Taxi service is available on

demand both at the Harlingen and Brownsville airports. One way taxi fares

range from $60.00 to 80.00 from the Harlingen and Brownsville

airports.

Rental Cars-In addition to the

shuttles, and taxi services listed above, both the Harlingen and Brownsville

airports have rental car companies on site to assist you with your

transportation needs. The Harlingen airport on-site rental car companies

include: Avis-(1-800-331-1212), Dollar-(1-800-800-4000),

Enterprise-(1-800-736-8222), and National-(1-800-227-7368). The

Brownsville on-site rental car companies include: Avis-(1-800-331-1212),

Exhibits

Multiple organizations will be in attendance to provide

registrants with information and materials for expanding health care services

to farmworkers. The cost to exhibit is $350. To register your
