Hi Joni

I just read over my Dr.s letter, and it is not all that detailed. I had my

OB-GYN write the letter since I have used her as my PCP for the last 4 years.

She described me weight , obviously, and the methods of weight control I have

been on in the past under her supervision. She wrote short 2-5 sentence

segments on each of the follwoing headings: Past medical history - only my

C-Section, otherwise I have no major history, Family History (again, major

things) Allergies (none) and Social History: basically that I am married,

non-smoker,occasional alcohol drinker, no drug abuse other then prescribed

drugs. A summary of my most recent exam. She then supported that she agreed I

should be considered for the surgery, adn that she would support me.

After I sent my packet in, Debbie called to tell me an OB-GYN could not be

the supporting physician . .which I was bummed about since she is the only

Dr. I have seen over the last 4 years! So, I went back to the Dr. I used

before her, which I didnt like too much, told her what was going on, showed

her my OB-GYN's letter as well as my 3 ring binder of research, she hand

wrote a note on a prescription pad stating that she is willing to follow me

after released by Dr. R and monitor lab work as indicated and she is

available for needed medical problems. I faxed this, and got scheduled the

same day they got that letter. The biggest part they want to know is that

there is a Dr to help you back home, and even more important is the letter

you write!
