Guest guest
Jan I printed out a blank copy and wrote in my information. i also made a copy to put in my packet (with information written in) and Dr. R approved my packet today. so the best thing to do is print out a blank copy and write in all your information and then go back and fill in the online form and submit it. Thats what I did. and yes if you don't have your patient information you will be filling it out again at clinic. I was lucky I had printed out one copy for myself and one copy for my packet and didn't have to fill out another one. in Kannapolis Patient information form > > Hello everyone, > I am learning so much from all of you and I am so envious of those of > you who have a date.
