Jan I printed out a blank copy and wrote in my information. i also made a

copy to put in my packet (with information written in) and Dr. R approved my

packet today. so the best thing to do is print out a blank copy and write in

all your information and then go back and fill in the online form and submit

it. Thats what I did. and yes if you don't have your patient information you

will be filling it out again at clinic. I was lucky I had printed out one

copy for myself and one copy for my packet and didn't have to fill out

another one.

I printed the confirmation sheet and then I used my browser back button and

went to the beginning of the form I had filled out and just hit print...no

problem!
