The entire FIN (Family Information Notebook---at the left of the screen) is filled with great information and forms for record keeping. I keep my own to document the things that might slip past the school or that they are afraid to tell us. Documentation is key to resolution in meetings. The Individual Health Plan link is to a form that a doctor/nurse fills out. It is well worth the trip to the clinic if your doctor will do it. Although Dakota has multiple health concerns requiring the form, our physician had no problem filling out the dietary specifications and since this has happened, absolutely we have had no problems with compliance (and even amazing acceptance and interest) at the school. Another link on the page has Other Information Regarding My Child's Medical Condition. This is a form filled out by the parents--this can include SCD and why and listed as an addendum to an IEP/504 Plan. This is an alternative to the physician form, but make sure that it is added to the IEP/504 Plan for your child's protection. Hope this helps.
