That's what I did. I put a post it in the envelope with the

I was reading in one of the recent posts that you need to choose a few

surgery dates that would work for you to send along with your packet. I

have not heard of this before. Any input? I am just about ready to send

packet in. I have my final appointment with my PCP today to finish

history, physical & support letter from PCP. I had an appointment last

week, we spent 1 1/2 hours filling out history that DR R requests. My PCP

jokes if we didn't know each other already we will certainly know each

other well after this! Will someone please let me know about the date

thing? On pins & needles.

Dear , I had never heard about that date thing before either,

and I have been on this list daily for months. I didn't send in

anything about my date but rather discussed it with Barbara on the

phone after I was approved. I know how stressed you are right now

since I have recently been there done that. Don't worry be happy is

easy to say but harder to do since this is so important to us.

Hey, I did not send any dates with my packet. I just waited to have all my

approvals (Dr. R, Ins.,etc) and then called Debbie for a date. She asked me

when did I want to go and since Wednesday was filled up, I took Thursday 8/10.

Don't know if this is the normal procedure or not but it worked for me!
