Guest guest Posted March 10, 2008 Report Share Posted March 10, 2008 Hi, I am working on a grant proposal for another course and have chosen to create a program for migrant follow-up care. My goal for the program is to decrease the " bounce-back " of patients as well as enhance the customized cultural care for the patient. The benefits would be monetary for the hospital and for the individual they would see both health benefits and monetary benefits. I was wondering if you knew of any already existing programs (doesn't have to be NH County, anywhere in the world) that would parallel to this or anywhere I may look to find information on any arena of this matter. I am being somewhat successful with my research however, I was hoping you may have some knowledge as to what may be helpful for me. Thanks for your time.
