My 4 month old twins (boy/girl) are getting their DOC bands next week. I have lots of questions....but I'll start off with my question about insurance claims. We paid the full amount and want to submit a claim to United to see if they will cover some of it. However I've heard that in the letter, there needs to be specific wording or phrases in order for them to accept it. Is that true? ANd if so...what do I need to say!??! Guest guest Posted March 25, 2009 Report Share Posted March 25, 2009 Welcome- Which CT will you be going to? We went to Charlotte and we had to file our own claim through BCBS (FEP). The claim was only one page and I had to fill in a couple of codes that were provided to me by one of the insurance specialists in the Phoenix office. They sent me a packet actually. You should ask your local CT office for help with that, and they should be able to either give you the proper codes/information, or direct you to someone at headquarters. I think the codes are universal. For diagnosis, I put 754.0 Plagiocephaly, and for the description of the charge, I put S1040 Cranial Remolding Orthosis. But there is another code I've heard of that starts with an L, maybe L1040, so you need to make sure you put the right one. I would double check on that one. Good luck!
