May take a few minutes to get, but it is YOUR file you own it and it will be worth your while. If they offer you a copy, tell them they may keep the copy. , Are you sure about this, that the original chart is ours? When I left my first GI and went to his office to pick up my records, all they would give me were copies of the most recent visits. I had been getting copies of the radiologist's reports and office visits all along, so there were only a few appointments that I didn't have copies for. When they gave me these copies, there weren't any doctor's notes or memo's there, and no typed summary like all my other doctor's do, just the one page handwritten record of weight, bp, and two or three word comments, like notations " pain, change medication " or " needs Miralax " . None of the doctor's that I obtained records from for my SSDI review gave me originals, only copies. All the other doctor's copies seemed very thorough, except my treating GI's, the one who wouldn't test me for diabetes when I was having all these problems last year when I went into pancreas burn-out and DKA. His were so brief it was as though there was information missing. I didn't know that we were entitled to have the originals. With hope and prayers, Heidi Heidi H.
