Also, this is where the gallbladder sits-- so with it being intermittent, it's possible that she's setting it off by what she is eating. The fact that her big toe is giving her fits also is suggestive of gallbladder-- or gout. It can also mean she has something out of place in her neck. does it mean she has sarcoidosis-- not necessarily-- but she needs some tests done to figure out what is going on. If she has joint or ligament pain other than her feet-- I'd get her into a rheumatologist as they can check for gout and immuno issues.
