It has the same effect as eating less, it's just enforced by body modification. There is a very high death rate for stomach surgery patients. Why in the world should we (and saying " it's insurance money " means " we " - where do you think insurance money comes from?) pay for a risky surgical procedure that substitutes for self-control in the patient?
