I have printed out my state disability forms and was wondering to whom we give the forms to? I received a pre op packet yesterday with how to prepare for surgery and it stated they do not fill them out. Do we give it to the surgeon to fill out? Any advice would be appreciated. Guest guest Posted April 21, 2006 Report Share Posted April 21, 2006 I had surgery in Fremont. The day of my preop appt. I went to Kaiser services there at the facility. They gave me the forms I filled them out, my dr. signed them and after my appt I gave them back to the Kaiser services and they filed everything for me right there. They didn't want to keep my papers, they wanted me to wait for them. I was just released by the doctor and I needed to go to sleep! I was tired from getting dressed. =0) They finally kept my papers and called me the next day and told me I could come and pick them up. They should have been sending them to Disability! I told her she needed to send them to me in the mail as I live in Napa and I am not coming all that way to pick them up. When they arrived they were not filled out properly, in fact they didn't fill out anything, they just attached a note from the doctor so Disability wouldn't accept them. I just forfieted disability. Thankfully I had enough time on the books at work and used it. To me it was more trouble than it was worth. I would suggest you go and talk to them BEFORE you have surgery and find out what their process is. Good luck. My husband filled out the part that we were supposed to fill out and then talked to Dr. Im to sort out the rest of the paperwork and then he took it down to the business office before we left on 3/7/06. The business office is just across from the pharmacy as you head to the B elevators. It was no trouble at all for us. My husband took care of everything for me. If you have a support person going with you, they can check in with the business office while you are in surgery and also get direction from the person at the surgery admitting desk. They will have a lot of time to kill, so they might as well being doing something useful for you. The papers can't be submitted though until the surgery is complete. I had enough sick time, but I didn't want to give up something I was entitled to, and who knows when you might need to use your sick time. The one thing that I am telling everybody who goes into the hospital is to take a pair of ear plugs, noise cancelling headphones, or your ipod. It was SO loud in the hospital. My roommate was an ederly woman who continually mumbled, and had her tv on until well past midnight, and though there were no monitors going off in our room, it seemed that they were going off in every other room.
