I vaguely remember pre-op for my gallbladder and the admitting staff asking specifically if I had any written, legal documents regarding my health care (I didn't). BTW, Nolo (a respected legal publisher for whom I do NOT work) notes that you can use a Living Will/Health Care Directive to specify that you WANT them to use all and every available measure to preserve your life.
