They will set up an appointment and generally it will be a phone interview. They will be getting basic information at the interview. If they believe your son may qualify for SSD they will send you a form to complete. This is where your proof will come in. They will ask for doctors names, addresses, phone numbers, etc. They will ask questions about the actual problem as well as limitations. SSA will actually get the information from the physicians. Now, something to consider here is that a diagnosis of morbid obesity and any other illness or disorder usually qualifies one for SSD. Include as much information as you can about other illnesses such as diabetes, sleep apnea, hyperthyroidism, elevated cholesterol, heart diseases, and even the back injury. When you have the interview, you will also want to stress these same kind of problems. You won't necessarily needs dates of diagnosis at the interview. The thing that helped me the most was making a list of these problems and even the medication I was taking. It helped with the interview and filling out the paperwork.
