Short Term Disability/? for Post-Ops

My short term disability form requests that the ICD-9 codes be

listed..I know the LOMN is submitted under ICD-9 278.01 - Morbid

Obesity, but I'd rather not have that on my short term disability

paperwork for work if possible. Does anyone know what ICD-9 code

their doctors put for their short term disability form ?
