question about ins/payment for surgery

Hi my niece who is 18 is preparing for this surgery. It is totally non

cosmetic. Her teeth and jaw have problems. I am sure you all know way

more about this than I do. However, my sister is having problems with

the Dr wanting a large sum of cash upfront. They have very good ins. Is

this normal? any info would be appreciated.
