As our transition, by all accounts was not good, I am trying to figure out what SHOULD have been done... We started seeing Dr. Herzenberg in Baltimore after Maddie was out of her casts and we were having a lot of problems with the DBB. We were seeing Dr. Sponseller at 's Hopkins. When she was casted, he used the peel away casts, not plaster. When she transitioned into the Markell shoes, I peeled her casts off on the day that he indicated (we were at home) and per his instructions, put her immediately into the shoes, 23 hours a day. He never saw her feet prior to the shoes, and he never fitted her. I went to the shoe store and the sales associate helped me... We (meaning me and the sales associate- after e-mail contact with Dr. Sponseller) tried three different pairs of shoes prior to seeing Dr. Herzenberg. What I am trying to find out is how does one properly transition from the casts into the brace (DDB or )? Maddie screamed almost non-stop for two weeks. This is why we sought out Dr. Herzenberg... Dr. Sponseller was not helping and even suggested that we reduce to 50 degrees abduction vs. the 70. I am trying to figure out what SHOULD have been done. Her feet and legs were so red and puffy when the casts came off, but Dr. Sponseller told me that she had to go into the shoes immediately, I mean right away. I was acutally scared that I may not get them on fast enough! She was sized (again, by the sales associate at the shoe store) with her casts on. As I was doing it... it made no sense to me, but figured Dr. Sponseller knew what he was doing. She screamed so bad when I would put them on and would not stop until she literally fell asleep from exhaustion. This is when I found this group.... and realized that something was not right. As Dr. Herzenberg has indicated to me that Maddie's feet do look good and are corrected properly. I still wonder what I should have done differently. Hind sight is 20/20, but perhaps others can benefit. It seems to me that the transition phase is important. How do you " do " the transition properly? Our doctor had the orthotist come to the appointment with us and she and the orthotist fit the shoes on together and both made sue that the shoes and bar were the proper fit when we left. Not all doctors do this but it certainly gave me great peace of mind during that adjustment period because I knew they fit correctly. I can't imagine having a shoe store lady set your kid up in the FAB. But I don't really know what to say because we took Everett's last casts off at home (applied by Dr. Ponseti) and there wasn't any of the redness or swelling I hear others talk about. He did not wear the Markell's well though and had issues with deep sores on his heels until we got the 's. After that it was smooth sailing. Theoretically the child's foot should be fitted to the shoe prior to that last cast so you have the right size. s. Guest guest Posted February 23, 2006 Report Share Posted February 23, 2006 Our first go at the shoes was by no means perfect, but through no fault of the doctor, or the orthotics department at Gilletted. On the day Aleksander had his last casts removed, we had three appointments: 1. With the cast techs to have the casts removed 2. With Dr. Quanbeck to check Aleksander's feet 3. With ATD to get Aleksander fitted for the shoes, and for my husband and I to go over how to put the shoes on. When we went to ATD, they measured Aleksander's feet and shoulder width, and then prepared the shoes and bar for us. They demonstrated how to put the shoes on, and then let us try it a few times before we left. Aleksander actually slept thru getting the shoes put on, and our trying them on him several times. We ended up having problems, and Aleksander came very close to developing pressure sores. But, we called Gillette and spoke to the doctor on call that day, even though it was a weekend. He ultimately ended up in healing casts, but the second go at the shoes has been just fine. Hope that helps.
