Guest guest Posted January 9, 2006 Report Share Posted January 9, 2006 Hi nna, Our daughter Emma had the ATTT done when she was 4 by Dr. Morcuende in Iowa. The procedure is relatively minor but requires 6 weeks of post operative toe-to-groin casts bent 90° at the knees. You can feel the anterior tibialis tendon by pulling up with your foot and feeling around the top, inner part. That tendon is shifted over to the top outer part of the foot. A small hole is drilled in the third cuneiform (a bone situated just above where the 3rd and 4th metatarsals begin) where the tendon will be transferred to. There is no bleeding since they use tourniquets to stop blood flow to the area. You're probably wondering about pain after the surgery--from what I've read some children have it worse than others. Emma had a tough time because the pain meds they gave during surgery never took and she woke up in pain. This is apparently very rare and I haven't heard of anyone else going through it. It seems that half the kids are able to leave the same day while others spend the night. In general, the first 24hrs - 36hrs are the worst. Even through the worst of it Emma still managed to eat well and have a few laughs watching a movie. Feel free to write with more questions or for clarification. Long and short is that she is now 20 months old and her left foot is starting to relapse. We saw Dr. Ponseti on Wednesday and he said that most likely Macie will need to have a tendon transfer around 2 1/2 years of age. We weren't prepared for this news as we are very strict with the brace and do all we can to prevent relapse. We are now having her wear her brace during naps as well in effort to stop or correct the new problem. With your experience I am curious to know anything and everything about the tendon transfer procedure and recovery. I have looked online and not had much luck finding non-technical information or for that matter any information on the tendon transfer. I read that you have posted several times about this and I am still trying to learn how to navigate this site so I apologize:-) Any information or links that you can provide will much appreciated. We have been and still are diligent in putting on Christian's shoes every single night yet his doctor believes that when we see him in March we will be scheduling his tendon transfer. Christian will be 2 years old on the 30th of this month. I find myself constantly trying to find more information about this procedure. Unfortunately, I was hoping for him to disagree with Dr. Herzenberg but I guess in some cases it has to be done. Guest guest Posted January 16, 2006 Report Share Posted January 16, 2006 will require the ATT surgery. I was shocked last month when my doctor told me because is only 18 months old. But after looking through some things I found my email from Dr. Ponseti. I had sent him pictures when was nine months old and he noticed that supinated his right foot and told me that with time this may correct itself. If it did not he would need surgery after the age of two. Through my reading I found that the reason this needs to be done after two is because they need to wait for a certain set of bones to calcify (sp) for the surgery to be successful. So my answer (this is to the best of my knowledge from the reading that I have done) to the question why this surgery as opposed to recasting is because it is actually not the clubfoot/bones that are the problem it is an issue with the tendons. Dr Dietz doesn't know exactly why that is; Jake did try to stand on his cast many times (being weight-bearing will often pull the tendon out) but there are lots of kids who walk and even run in their casts without a problem. In any case, Dr Dietz suspected that the tendon pulled out because Jake had a lumpy area on the top of his foot around where the tendon was inserted. The symptoms Jake had which told us something was wrong, was that he couldn't lift his foot upwards (the tendon wasn't working well enough), and he also walked on tip-toes. The toe-walking got progressively worse to where he couldn't walk flat-footed. Jake never appeared to be in any pain though. Most children do not need the ATTT done again. Dr Dietz told us that he has had to re-do the ATTT on only something like 6 kids in over 20 years of practice. Yes, we had the same surgeon both times. Dr Dietz is at the University of Iowa and works with Dr Ponseti. We asked about getting a second opinion, and Dr Dietz said that any surgeon in his practice would say the same thing. We considered briefly going elsewhere for another opinion, but there really isn't anyone else in our area doing this surgery or with as much experience treating clubfoot. We'd have had to go to Minneapolis or Milwaukee, where our insurance likely wouldn't have covered any of the treatment. Hope this helps! Let me know if you have any questions. At the time I had asked the group which pictures to send. I would guess that because the tendons are not as strechable as muscles is the reason for the surgery. Most of our children have the tenonomy done because the tendon does not strech enough for the heel to seat properly in the shoes so my theory is the ATT is done for similar reasoning. His foot doesn't necessarily pull to the clubfoot position it almost looks like he is standing a cigar and his foot curls with the shape of it. I will send Dr. P more pictures as time goes on, closer to the surgery, but I had actually noticed that rolls his foot to the outside while walking. It has gotten better since he is becoming more mobile but it is significant enough that others have noticed it with out my pointing it out.
