the WLS surgeries require changing the stomach in some way (the RNY cuts or

staples it horizontally into two parts; the DS removes about 2/3 of it ---

the fundus; a VGB includes inserting a permanant band into the stomach

wall)... Do you mean having the intestinal portion performed without any

change to the stomach? I'm not sure if this would be workable because

although the DS is primarily absorbptive, the reduced stomach size does play

a role in helping curb appetite and reduce intake. The BPD surgery allows

for a larger stomach, but the lower portion (the antreum and pyloric valve)

are removed entirely.

The only surgery I know of that doesn't involve surgical manipulation of the

stomach (or minimal manipulation) is the new lapband. It is also the least

invasive of the surgeries, although long term results so far haven't been as

impressive as the DS. However, the lapband (filled with water to reduce

stomach size or deflated to make more room) can be readjusted (without

further surgery from what I understand) so if one starts regaining weight,

one can lose again and accomodate.

I would definately look into this possibility since you seem to be severely

restricted in your choices due to your stomach condition. I don't think

there are many surgeons who are performing the lapband (it only recently was

approved), but I know that Dr. Ren of NYU here in NYC does perform

it. I don't have her website address (can anyone help?), but I'm sure there

would be more information there about the procedure. There are also various

e-groups devoted to the lapband and those pre-ops and post-ops who want

to/are thinking of/have had it done. Perhaps Dr. Ren's website has links to

these...

Hi Pam -

You might want to run this by a DS physician. They may have a very

different take on whether or not you can have additional surgery on your

stomach.

Pam,

A study was done in Italy on just this procedure. The results were

moderate weight loss an big improvements in comorbidites like

diabetes and high cholesteral. So they weight loss may not be

entirely what you expected, but the improvements in co-morbidites

make it worthwhile.

Actually you can't really have a " DS " without stomach surgery. What

you would be having is a Bilopancreatic diversion without gastrectomy.

In lieu of the gastrectomy your surgeon can consider other

restrictive procedrues such as the lap-band in addition to the BPD if

BPD alone is not effective in reducing your weight to the degree

required.

Alternitvely you can go with the RNY which bypasses the stomach

entirelly, though that would not by my preference (or anyone else on

this list I suspect).

I would assume that you could have the intestinal portion of the DS,

the only concern may be the proximity of the duodenal stump to the

stomach--it depends what exeactly you had done to the stomach.

Hope there is a good chance for you to get this part of it.

Dear Chris: I am always amazed (and impressed) with

your command of the medical issues and the clarity of

your descriptions and explanations. Come on now, are

you really a doctor masquerading as a layman? Since

I'm new here, I'm probably not as familiar with you as

others and you may have already shared your source of

knowledge but I'm not familiar with your background.

Anyway, I think you're a wonderful asset to this site.
