My son's speech therapist suspected it at 18 months and that was confirmed by his next few therapists. It is an oral motor dyfunction of some sort. The kids can't figure out how to correctly position their tongue, mouth, teeth, etc in order to produce the right sounds. Danny has been getting speech since 18months and is now 5 1/2 and is much clearer but still not always understandable. He was barely understandable until he was 3 1/2. This has really made the most difference. Linguisystems has a really good book on apraxia. I have seen great improvement with Just for Kids: Apraxia. by Martha Drake.
