I know that these wonderful gals(and guys) will help you out. We

have all BTDT, and know how you are feeling.

So, first...she has Markell's? Are they open heeled or closed?

Most pressure sores appear to come from slipping, so you probably

need to position the foot different IF the shoes are the right size

(my son's was given the wrong size). Kori, ...anyone out

there more knowledgable than me at the moment?

Are you sure her feet are corrected enough at this point to even tolerate the

shoes? You're right to be concerned about the sores coming on.

The 's are an option. I've been using them on my youngest for 2 years

now and love them, never an issue with sores and his feet are holding their

correction beautifully. Sometimes they are better for children who had less

than adequate treatment such as yours started out with.

There are other things though you might want to check in to - the size of the

shoes, the angles they'r set at, the condition of the foot itself....

socks, using moleskin and blister bandaids and it doesn't seem to

really make any difference. I'm probably a little paranoid because

our daughter was not casted properly by the first doctor and ended up

with a pressure sore from the cast that developed into cellulits and

she had to hospitalized and on IV anti-biotics! And I don't want to

go down that road again!

doctor said that the s are more of an " ankle treatment " and

he wasn't sure about the long-term success rate for cf. However, I

haven't seen that mentioned in any of your wonderfully enlightening

also hate to see her so unhappy and uncomfortable!

Hello ,

Welcome to the group! First off, sorry to hear that your daughter has

had to deal with improper casting/pressure sore! I don't think that

you are at all being paranoid by worring about red marks on her feet

and her uncomfortability in the shoes, she should have adjusted by

now if her feet are properly corrected, the brace is set up right and

you are getting the shoes on properly...we will all help you get this

figured out so your little girl is comfy and happy =)!!

First off a bit of background on her treatment would be helpful. When

you say she had unsucessful casting, what did that involve (how many

casts, were they full length toe to groin and what kind of results

did you see). Was her last cast rotated outward to a 70 degree angle

and does she have good dorsiflexion (the ability to bend the foot up

towards the knee), it should ideally be 15-20 degrees.

How is her brace set up? Her club foot shoe should be set out to a 70

degree outward rotation (this will not be comfortable though if her

last cast was not at 70 degrees) and her non club foot should be at

around 45 degrees (our daughter is unilateral also - right).

We really need to figure out/make sure that her foot has been

properly corrected. If not, she will never be comfortable in the

brace and will probably end up with more sores/blisters which is what

we DO NOT want to see! And also we neeed to make sure that her brace

is set up properly. Who is her current doctor btw?

It would be really helpful to see pics of her feet and her brace to

help us determine if all is OK. We are not medical doctors but many

here have a very good eye and will know if her foot is corrected or

not. There is also a nurse who worked directly with Dr. Ponseti named

Beth here who's advise and knowledge have been invaluable, it would

be really good for her to see your daughter's foot.

You will need to join the CFPics site as this site does not support

attachments. There is a small survey to be filled out and sent back

and then one of the moderators will approve you so you can send off

You should take the following pics:

*One directly from the back with her standing and bearing weight

*One from the front with her standing bearing weight

*One from the top showing her foot at rest

*One dorsiflexing her foot (cup her foot and apply pressure with your

thumb and get her foot to go as high up toward the knee as is

comfortable possible)

*One from the bottom showing the sole of her foot

*One with her wearing her brace from the front (try to take the pic

so that we can see the rotation of her shoes and distance appart they

are, take a few if needed)

This will help ease your mind that she is fully corrected and that

her brace is properly set up so that you will only need to deal with

getting the shoes on properly.

Some kids do have a real problem tolerating the Markell shoes (our

daughter didn't) it's usually kids that have atypical/complex

clubfoot and this is why the shoes were designed and made.

Not sure where your orthotist got his information from but it is NOT

correct - they were specially designed by with Dr.

Ponseti's help for our kids.

Glad that you found this group, you will find the parents here so

caring, supportive and informative! As I said, we will all put our

heads together here and help you get this figured out!

It has already been such a blessing to find this group! Your

response have brought tears to my eyes as I don't feel so alone in

this very confusing process! Thank you!

Okay....our daughter's name is Mallory and she is 3 1/2 months old.

I will definitely join the pix site and send the pictures you

indicated...any assistance would be greatly appreciated!

We did not know that Mallory had a right club foot until she was

born. She had severe jaundice and had to be admitted to NICU.

Because we didn't know anything about club foot or its treatment we

went with local ortho who said he knew what he was doing but he

obviously didn't! She had 2 casts put on by him...the first stopped

below her knee and the second was full length. He put a thumb

imprint on the outside of her casts on her ankle which is what caused

the half-dollar sized sore!

Her current Doctor is with Riley Hospital in Indianapolis and his

name is Dr. Randall Loder. We have been very happy with him and his

treatment. She had a total of 5 casts with him (not including the 3

weeks cast after surgery). She had the tendon surgery and final

cast 3 weeks ago. Her last cast was at 70 degrees. She does

have good dorsiflexion but it is painful to her and makes her

cry...is this normal? I just checked her brace and in my very

limited opinion it looks to be 70 and 45. Is is normal for the sore

areas to be on the non-club foot?

Our original orthotist told us the same thing about the

s...That they were more of an ankle treatment...Mine was

confused and turns out didn't really know much about them.

More than likely, your daughter is just experiencing her adjustment

period to the Markells. Hang in there...it will probably get better

soon. Check the bar length, like said. If the bar is

adjustable, make sure that bolt to bolt, it is the width of her

shoulders. Our orho didn't believe in that theory (of bar being

shoulder width). did not have an adjustable bar, and they

wouldn't get us a longer one. The width can make a huge difference!

We also thought that our daughter was fully corrected, but after

taking her to Dr. Ponseti and letting him recast her, we now know

that she was never fully corrected before when she was not

tolerating the shoes. Her feet looked straight to us, but she was

never able to put either foot flat on the floor. Dr. Ponseti took

care of that. She is now in the s and they are wonderful.

Sorry I don't have a lot of time to give you right now, but rest

assured that we can help you get to the bottom of what is causing

these sores for your daughter.

I mainly wanted to say that the brace was designed

specifically to hold the correction of a clubfoot and nothing else.

It has nothing to do with ankle treatment. Your brace guy has no

clue! : )

I'm sure you'll get lots of feedback here and I can chime in more later!

I just realized you wrote that the sores are on the normal foot, not

the clubfoot. What is the angle at for that foot? Should be around

45 degree.

The sides and the ball of the foot, hmmm, is it possible that the

normal foot needs a different size shoe, like a regular or narrow?

Has she been slipping in them or out of them entirely?

My original orthotist had no clue either.

We will figure this out, ok!

has it right on here, shoe angle first - is her non-affected

foot set to somewhere between 35-45 degrees? If it is set for more

than this it is too much. Can you tell me more about the sores?

Just red areas? Any broken skin? Or are they dark purple deep like

the pressure sore she got from the cast?

Shoe size: you can download the Markell shoe size chart from here -

My understanding is that the foot should match up width wise with the

chart for a Medium width shoe. If your daughter's foot is wider than

this, she should have a wide width shoe. If they are pressure sores,

the fact that they are on the ball of her foot and sides would

indicate that this is likely the issue.

The other thing that may be a problem is the bar itself. Not only

bar length which could be playing a role here too, but the bend in

the bar. If the bend is too severe it will cock her unaffected foot

at an odd angle. Actually the unaffected side does not need a bend

in the bar at all, it depends on how your orthotist bends it - one

curving bend will automatically bend both sides. If they do a bend

at either end then you don't even need it bent on the side with the

unaffected foot. Tell us more about how your bar and shoes are set

up and we can certainly help you diagnose this! Hang in there, we'll

help you get your baby back to being a happy girl. : )

Could I ask who your first doctor was? We are from Indianapolis, and switched

doctors because our first altered the bracing. I often noticed thumb prints in

's as well, but he never got sores. Just thought I would see if it was

the same doctor. We switched to a certified doctor in Cinnicinnati because we

wanted to make sure everything went right the second time. I am glad you are

having success with a local doctor.

Are you sure it hurts her to dorsiflex? resists a lot, but have very

good flexiblity. Could it be that she just doesn't like it?

As for sores: She shouldn't have them anywhere. Do you think her shoe should

be a different size on the nonclubfoot like maybe a narrow width.

Welcome aboard-our son was treated at Riley hospital and we

sometimes still get his brace/boots from the brace shop. I love Dan-

however his coworker is not so very nice-just didn't have much of a

soft touch towards our little man. Dan made sure before he sent us

home that we knew what we were doing and that they fit properly-the

other guy handed us the new boots/bar and sent us out the door-

without even trying them on!! Ugh. Our doctor is not at St s

but like I said-I still like going to Riley-i love that place! Sned

us some pictures so we can see whats going on with Mallory. If its

just her ncf foot-my bet is the shoe is either too big or too small

for her(more likely too small)-some ucf need 2 different shoe sizes.

We live in sunny south bend!

Sorry it took so long to respond....our first Doctor was in Fort

Wayne....Dr. Karr....he was the one who put the thumbrpints that

caused the cast sores. We then switched to Dr. Randall Loder at

Riley and we have been happy with him. Are you familiar with him?

We actually got his name from the Head of Orthopaedics at Children's

Hospital in Cincinnati! Are you happy with your doctor in

Cincinnati? Just curious about options around us!

Kathleen,

We too have been very happy with Riley but until this bracing

process! We are from Fort wayne....not too far away from South

Bend! Who is your Doctor at St.V's? Are you familiar with or have

you had any experience with Dr. Loder at Riley? We like him and

believe he knows what he is doing but we are also very new to all of

this and just want to make sure we are doing what's best!

We are very happy with our doctor. I am not familiar with Dr. Loder. Has he

been at Riley for a long time? Good to know there are local doctors in case we

needed to swich for some reason!

Hospital in Cincinnati! Are you happy with your doctor in
