Guest guest Posted November 13, 2005 Report Share Posted November 13, 2005 It is possible your Logan is losing correction while he heals so when you put him back in the shoes, even the right shoes, the foot may not fit well and again cause sores. Have you considered a " Healing " cast, to hold his correction while he heals up? s. blisters Hello, I am new to the group and looking for advice. My son Logan was born 8/11/05. Despite several level 2 US, we did not know about the unilateral club foot until he was born. We had not heard of club foot and were led to believe it was no big deal, just a few casts and all would be well. We went to the ortho recomended by our ped and felt he was quite knowlegeable. He told us about the serial casting and the posibility of his heel cord being cut. We did about 4 weeks of casting, then the heel cord was cut. After that cast came off we were fitted for shoes. Logan screamed for about 24 hours with the FAB, then was generally unhappy for several days. We had trouble figuring out if his heel was down and back and he was developing a blister. We went to see the orthotist twice to get lessons on the shoes. She saw that he was developing a blister. I asked for the shoes with the open back that I had found on the Markell site. She ordered these, however by this time there were several blisters on both feet-seemingly from the padding added by the orthotist and stitching on the back of the shoe. One foot was so severe that we had to stop with the shoes and he has a prescription to help heel the blisters. While with the orthotist, she could not figure out why 2 pairs of the same size shoe looked so different. I contacted Markell and discovered that the shoes we were given were quite out dated, and had we been given the shoe currently available we could have avoided this. So Logan has been out of the shoes since Thurs and we see the ortho on Tues. I was just told to do some stretching with his foot. I am worried that we are losing ground and he has suffered unnecissarily. We are in CT and there are no dr's on the Ponseti list. Guest guest Posted November 14, 2005 Report Share Posted November 14, 2005 , Honey it's a big mistake to start at 45 and " move up " to 70 degrees. The process just doesn't work that way. The natural tendency of the cf is to revert to it's cf state - so if you are starting at 45 degrees, that foot is fighting against the 45, trying to turn back in to it's cf shape, therefore trying to wrench it back out to 70 with the shoe is a fight you won't win. The casting process brings the foot out to the 70 degrees - what is called over-correction - because with time, the foot will gradually revert back a little bit (once the FAB is discontinued) to a normal state of correctiveness. Just keep in mind the FAB is used to HOLD correction, it is not designed nor prescribed to GAIN correction. If you go from 45 to 70 degrees with the shoe, you are asking that shoe to gain correction and it's just not built for that. The FAB will only hold the correction the foot had prior to it's use...and with out proper use, it will not hold that much, thus the foot relapses and you've got trouble. I've seen the kind of sore you are talking about, years ago on my oldest cf son. His heel was hamburger with deep black-purple-red infected holes drilled deep in to the heel. This ws prior to knowing about Dr. Ponseti and stuff like that...but the FAB tore him to shreds. I know now it damaged his foot because his foot was not ready for the FAB - it was like putting a square peg in to a round hole and expecting it to fit. More than likely your child will need re-casted to re-gain his proper correction before you use the shoes again. You may consider biting the bullet and buying the shoes if this continues, or try cutting the heel out of your Markell shoes first. Mostly though, be sure the feet are ready for the FAB before you start using it. Hang in there, I promise it'll get better eventually! s. Re: blisters Hi, and welcome to the group! Is his foot abducted to 70 degrees of outward rotation? Do you feel like his feet were fully corrected? Often the cause of blisters and sores on the feet are not just ill fitting shoes, but feet that are not fully corrected. It is possible that he may not relapse in these few days but your doctor should definitely look at his foot before just getting you the new shoes and sending you on your way. If he sees that the foot is indeed relapsing he should do another cast to regain correction prior to putting Logan back in the shoes. If you are looking to get a second opinion, I don't know of any Ponseti doctors in Connecticut, but there are several in New York and Dr. Herzenberg in Baltimore is one of the best. We see the dr tomorrow and I plan to ask for him to be casted, as now Logan is not used to the 70%. Thanks for the advice. Logan's sores sound similar to what happened to my son, we went through the exact same thing. I would just be firm with the doctor, even if he tries to tell you the foot is not relapsing - if you are noticing his foot turning in plus the fact he was not casted to 70 degrees to start with is a definite reason to get another cast on him (or two if necessary). She is not on the Ponsetti list, but works in conjunction with Dr. Kasser, who is on the list. I am still worried about Sammy's heels, his sores, especially on his Non-CF foot were really bad and you can still see the scars from those sores. He's still only 13 months so I keep hoping that they will fade but I do worry about it. I kick myself daily for not being smart enough to keep that from happening, and I worry about that scar tissue. Guest guest Posted November 16, 2005 Report Share Posted November 16, 2005 His 1st (non Ponseti) tenotomy left bad scars, three in a row up each heel - then the FAB came along and sort of chews those lump/scars off his heels and did their own work, so ya, he has scars on his heels but over time they have lessened quite a bit. No other shoes have ever torn him back open, so it's not a factor anymore. s. Guest guest Posted November 16, 2005 Report Share Posted November 16, 2005 Hi , So sorry to hear that you and Logan are having such a difficult time with the Markell shoes!! It may be that Logan will need the 's but it also may be that Logan's feet were never properly and fully corrected, you will find this out when you see Dr. H =). I think it is very wise of you to take Logan to see Dr. Herzenberg! He is one of the best and it sounds like at this point, Logan really needs his expert hands! Big red flag for your doctor to tell you to start at 45 and work up to 70 degrees in the shoes!!! The shoes are NOT meant to do that at all, they cannot possibly function that way. Look at how the casts go all the way up the thigh, and are worn for a solid week, this is the only way to bring the foot to the desired 70 degree outward rotation. For your doctor to tell you that you could use the shoes to bring Logan from 45 to 70 degrees is just NOT right!! This will cause him more discomfort, blisters and will just not work! We are really glad to hear that you are seeking a second opinion with Dr. Herzenberg! Be sure to tell his asst. what has been going on and that it is very important for Logan to be seen right away!! My 5 month old was seeing a pediatric orthopedic surgeon who was highly recommended and something told me that I needed a second opinion. I am so glad I went for one! We are seeing Dr. Herzenberg now and I can not say enough good things about him. He follows the Ponseti method exactly and has done wonders with my son's bilateral clubfeet. Here is their website http://www.shrinershq.org/shc/springfield/ It may take a month to get an appointment, but don't settle for any other doctor there, Dr. Mack is the only Ponseti doctor there. Depending on how far away you are you might want to consider Angel Flight, http://www.angelflightamerica.org/index.php They will fly you there for free and Shriner's will arrange a van to pick you up at the airport and take you back. They are really wonderful there and everything is free. Guest guest Posted February 2, 2006 Report Share Posted February 2, 2006 Amber, if it's " just " a blister, there are things you can do while still using the FAB to protect the foot and avoid relapses...but if it's the start of a pressure-sore, that's another matter. Have you looked at any of the tips documents in the files/links? They can help you sort it out with ideas. s. Blisters Keegan had his last cast removed on January 24th, and had the Markell shoes with the DBB put on the very next day. I noticed tonight while I was bathing him that he has a blister on his right heel. It's 11pm here, so it's too late to call our doctor. We called the health line, and they don't know very much about clubfoot, so they weren't much help. All she said was to put a sterile bandage over it. He's been a lot more cranky lately, and it's clear to me now as to why. I don't want to put the shoes back on, and so far from what I have read, I shouldn't anyways. Not until it's healed. So what am I supposed to do? Just let his poor little feet rest overnight, and then call our doctor tomorrow? Gah. If it's not one thing, it's something else with us. Guest guest Posted February 2, 2006 Report Share Posted February 2, 2006 Amber, Blisters are caused by friction. This means his shoes are not tight enough. Now that he's got a blister, it isn't the best option to keep putting them on, especially if you're not getting them tight enough. Some blisters can heal while in the shoes. But you MUST make sure that you're getting them on properly and the heel is deeply seated in the shoe. Here's an option for you. Put a bandaid over his blister. Then mark on the bandaid with a water based marker (crayola works great) so that it's wet and open the shoe up completely. Then lay his foot into the shoe and make sure the heel is deep down in there, press into the back of the shoe. With the shoe completely open, this will show you where his foot should be if it was seated properly. Then, look inside the shoe and see where the mark is. This spot can be removed from the shoe with a sharp knife (exacto, razor blade, etc.) and you can put the shoes back on and if you can get that foot seated properly the area that may rub the blister is gone. The blister heals. you have a holed shoe but that's ok. They will hold up fine. Have you read the tips .pdf? If not, read that and follow the instructions carefully. With the hole, you can see inside the shoe to know if the heel is down. Once you get that, you should be able to get one or two holes tighter on the strap. At this point, make sure the shoes are tight, tight, tight. LMK if you have any other questions. I will email you the file here in a second so you have it. hang in there! It wont go away,but keep on padding the feet cause they can turn into sores/blisters. I use MOLE FOAM. More padding. I also use 2nd Skin Burn Pads over the red marks, and Kling-Gauze (a special made top gauze tahts real soft like cotton). Let me know if You find something that works better!
